Completing FWPE Form - For Fieldwork Educators

This article helps Fieldwork Educators complete the FWPE (Fieldwork Performance Evaluation) form easily and efficiently. 

This article provides step-by-step instructions to support a smooth transition from Formstack to the new system.

Steps:

1. Accessing the Form
   1. You will receive an email from the school or the program with a link to the FWPE form. 
      
   2. Click on the link in the email "here", to open the form in the browser.
   3. If the link does not work, then copy the link in the email and paste it into a browser window. 
       
2. Fill the Form
   1. Complete all required fields carefully and check mark the attestation “Student was informed of this review”.
   2. You can do one of the following in the form.
      1. Click Save if you need more time and want to finish later. Use the same link to return.
      2. Click Clear All if you want to start over.
      3. When you’re done, click Submit (top right corner).
         
3. Submit Final Evaluation
   1. You can use the same link to submit your final evaluation when due.
   2. Bookmark the link or ask your school to resend it if needed.
   3. Mid Term Evaluation responses and score will be visible while filling out Final Term form.
