Accessing and Navigating the Surveys App
This section walks you through accessing the Surveys App and navigating its core features so you can begin creating and managing surveys efficiently.
Accessing Surveys App
- Login to the Exxat Prism using your university credentials.
- From the dashboard or hamburger menu, go to the Program section and click Surveys.
You will land on the Surveys App Home screen.
Navigating the Surveys App
This section provides an overview of the Survey Home screen and its key navigation elements.
The Survey Home screen includes the following key elements:
Home Button
Click the Home icon on the left navigation panel to return to the Survey Home screen from anywhere in the app.Form Library
Select the Form Library icon to access all forms available for creating surveys.Search Bar
Use the Search Bar at the top of the page to quickly find surveys by name.View Options
Switch between Grid and List views using the view buttons. This helps you choose how surveys are visually displayed.Sort and Filter Controls
Apply sorting and filtering to organize survey results based on your preferences (e.g., status, date).Create Survey
Click Create Survey to start building a new survey.Profile / Settings
Open your Profile menu on the top right to manage account settings or log out.-

Create Surveys
To create a new survey follow these steps.
Click Create survey. from the Home screen.
Choose one of the following.
Start from Scratch
Copy an existing survey
Create Survey from Scratch
Select Start from scratch tile.

Enter the Basic Details and Click Next.
Survey Title: Name for the survey
Survey Description: Provide the details of what the survey is and why the survey is conducted.
Closing Date: The date when the survey ends.
Access Settings: Visible to all program users or only you; cannot be changed after publishing.
Add Cover Image: You can upload PNG, JPG, or JPEG files up to 10 MB, to convey the branding or theme for the survey.

Design your survey and click Next
You have three options to create the survey.-
Start from Scratch

Click ADD NEW SECTION
to add a section .
Update the section title and description in the Attributes pane..
Click Add Questions
to add survey questions to the section.
Select the question type.

Sl. No. Question Type Description 1 Single Choice Question Allows respondents to select only one option from a list of predefined choices. Use this when you need a single, definitive answer. 2 Multiple Choice Question Allows respondents to select more than one option from a list. Useful for questions where multiple answers may apply. 3 Short/Long Answer Question Provides a free-text field where respondents can type either a brief response or a detailed explanation. Use this for open-ended questions. 4 Title Adds a non-question text heading within your survey. Use this to label sections or provide context without requiring a response. 5 Number Collects numeric responses only. Ideal for capturing values such as age, quantity, or scores. 6 Select from dropdown Displays a list of options in a dropdown menu, allowing respondents to choose one option. This helps keep the survey clean when you have many choices. 7 Date picker Allows respondents to select a specific date from a calendar widget. Useful for collecting dates of events, submissions, or schedules. 8 Date Range Picker Allows users to select a start and end date to capture a date range. 9 Time Range Picker Collects a start time and end time within the same day. 10 Toggle Provides a simple on/off or yes/no switch for binary responses. 11 Attestation Displays a statement that respondents must read and acknowledge. 12 E-Signature Allows respondents to provide an electronic signature for verification or consent. 13 Divider Inserts a horizontal line to visually separate sections or groups of questions. 14 Text Editor Adds a formatted text block for instructions, descriptions, or additional content. -
Enter the Question text and configure its attributes in the Attributes pane.
Click Save.
Click the settings
to add instructions for the survey.
Repeat for each question and section.
2. Use a Template
Click Use a template.
Select a Form Template
Review the questions and edit if needed, then click Save
2. Upload a Document:
Click Upload Document
Upload a Word or PDF file, the system will converts the document into a survey form.
Review and edit the converted questions, then click Save.
4. Distribute Survey
You can distribute Survey in any of three ways.

Via Exxat Prism: Distribute the survey to Exxat Prism users in your program.
Click Select Recipients in the Via Exxat Prism tile.
Select the user persona - Student / Faculty / Personnel, you want to distribute the survey to.
Select the students / faculty / personnel from the list. You can also filter the recipients list by applying filters in the Configure Filters section.
Click Add / Update Recipients

Click Email Template to select the email template to send the survey email
You can pick the email template from the saved templates
or-
You can create a new email template.
Click Add template to add a new template.
Enter a name for the template, Add Subject line, Merge Fields and Description.
Choose if you want to make this template a default template.
Click Save Template.

Additional Email: Distribute the survey to Exxat Prism by entering email ids.
Click Add more recipients in the Additional Email tile.
Enter the recipient’s first name, last name, and email address; or You can upload the CSV file with the email address.
Download the CSV Template > Fill the file with recipient details and email id > Upload the CSV file.Click Add / Update Recipients.

Anonymous Link: Distribute the survey via email or different social platforms.
Click Generate Public Link in the Anonymous Link tile.
Copy the Survey URL or QR Code to distribute or share.
After selecting the recipients, click Next.
Review and Confirm the survey details, then click Publish.
The Survey is now live. You can start collecting and analyzing the responses.

Choose an existing survey from the list to copy.
Review and edit the Design and Distribution steps.

View Analysis
You can view the responses and reports of the published surveys.
- Click View Analysis on any published survey to access analysis
- You can analyze the survey in the following ways

Insights: Provides key visual analytics that help you understand survey performance, response trends, and participant behavior at a glance.

Survey Status Overview: Shows the current status of the survey, including how many responses are completed, pending, or not started.

Weekly Response Rates: Displays how many responses were submitted each week to help you track participation trends over time.

Response per distribution channel: Breaks down responses by the channels used to share the survey, such as email or link.

Assignment Completion Time Distribution: Shows how long respondents take to complete the survey, helping you understand average and outlier completion times.

Survey Status Overview: Shows the current status of the survey, including how many responses are completed, pending, or not started.

Question Summaries: Provides a breakdown of responses for each question, helping you quickly understand patterns, trends, and overall respondent sentiment.

Reports: Provides detailed insights from your survey data, including raw responses, aggregated summaries, and downloadable formats to support analysis and decision-making.
You get two types of reports Raw Report and Aggregate Report. Click on Export Raw Report or Export Aggregate Report to download the report.-
Raw Report: Shows all individual survey responses exactly as submitted, allowing you to review detailed, respondent-level data.

Aggregate Report: Summarizes responses across all participants, displaying totals, averages, and trends for each question.

Templates can be reused when creating future surveys.

User Settings
You can switch between Light Mode and Dark Mode.
Go to User Settings.
Select Dark Mode or Light Mode.
Click Log Out to exit the App.
