2. What is captured under Additional Profile Details?
This section is fully customizable. Many programs use it to replace the traditional field application form
and collect key placement-readiness information.
3. How can faculty track communication and concerns for students?
Faculty can use the Communication tab for advising notes and field visit documentation.
The Interventions tab captures concerns, and both sections allow attaching documents
or email records.
4. Where should the Field Application Form be set up in EXXAT?
The form can be created as a simple questionnaire under:

5. Can we collect site preferences for each course separately?
Yes. Programs can:
• Create a Wishlist Questionnaire for each course, or

6. What is “My Requests” in EXXAT?
“My Requests” allows students to search available sites and submit placement requests directly.
Administrators can approve or reject requests, and create placements based on student submissions.

10. Is there a section in EXXAT to upload the Field Instructor’s (FI’s) license and certificates?
Yes. EXXAT allows programs to upload and store FI licenses and certificates under the
Personnel Profile within the Agencies module.
