In Exxat Prism the CI (Clinical Instructor) Details form is used to collect and manage information about the Clinical Instructors associated with the site or rotation.
This form ensures all the information about the instructor is documented, verified, and is accessible.
CI details form is used to assign instructor while scheduling the rotation. CI details form is also used to manage licensure renewals and check instructor eligibility.
To complete the CI details form for a course, follow these steps.
- Login to Exxat Prism and Navigate the Coursework section.
Click on the View All Courseswok in the My Coursework section from the dashboard. Or Use the hamburger menu to go to the coursework page. - In the Coursework page click on the course for which you want to submit the CI Details form.
- Ensure the Clinical Instructor is added to the course. If not you can add the clinical instructor to the course.

- Click the "+" icon in the Clinical Instructor section. You can add multiple Clinical Instructors as required.
- Click the "+" icon in the Clinical Instructor section. You can add multiple Clinical Instructors as required.
- Complete the Form and Click Save

- In the Course Activities tab, click on the CI Details FORM under the Forms/Evaluation section.
- Click on the Add Form button, select the Clinical Instructor to provide details for, and click Continue.
- Once the form is added Click on the Get Stared in the STATUS column to fill and submit the form. You can add multiple form in case you have more than one Clinical Instructor for the same course.
- Fill the form and Click Submit.

Trouble Shooting Information
- Why the CI Details Form Appears Blank
When you click CI Details Form, the screen may appear empty if you have not yet initiated a form.
How to Start a CI Details Form
Click + Add Form.
Select a CI Form from the list.
The form will now open and allow you to enter details.
Note:
You can add multiple CI Details Forms by clicking + Add Form again.
- CI Not Visible While Adding a Form
If you click + Add Form, and the system does not display any CI names to choose from, it means no CI has been added yet.
How to Add a New CI
Follow the steps below to create a new CI:
Click the ☰ Hamburger Menu.
Go to Coursework.
Select your Course.
Click the + icon to Add CI.
Once the CI is added, return to the CI Details Form screen and click + Add Form again.
- How to Submit the CI Details Form
After you add a form by selecting a CI, the system creates an entry that appears in the CI Details Forms list.
Steps to Submit the Form
Locate the CI entry you just added.
Under the Status column, click Get Started.
The CI Details Form will open.
Enter all required information.
Click Submit to send it to your school.
- Why You May See Only One CI Form
If you have filled more than one CI details form, but only one appears on your screen, it could be due to the Status filter.
The system may automatically show only the forms that require your action (for example, forms with status In Progress).
How to View All CI Details Forms
Go to the Status column on the CI Details Forms screen.
Adjust the Status filter to display all statuses.
Example: Select All, Submitted, In Progress, etc.
Once the filter is cleared or updated, all your CI forms will be visible.
