SEFWE form, also known as the Subjective–Environment–Functioning–What is the problem–Expected outcomes framework, is a structured documentation and clinical reasoning tool used in Occupational Therapy (OT) programs. It helps students organize client information and apply clinical reasoning in a systematic, evidence-based manner.
SEFWE forms play an important role in OT education because they support accreditation requirements by helping programs demonstrate student competencies in areas such as clinical reasoning, evaluation, documentation, intervention planning, and client-centered, evidence-based decision making.
They also promote consistency across fieldwork placements, as many programs require SEFWE forms for each new site to ensure standardized documentation, consistent evaluation practices, and reliable information for ongoing program improvement.
Exxat PRISM includes two types of SEFWE forms:
SEFWE I – Site Evaluation
SEFWE II – Educator Evaluation
SEFWE I – Site Evaluation
SEFWE I is used to evaluate the fieldwork site and capture the student’s experience during Level II Fieldwork. It reviews key components of the OT process including evaluation, intervention, and outcomes, along with environmental conditions, documentation practices, and caseload management.
The form also assesses the level and quality of supervision, summarizes the student’s overall fieldwork experience, and ends with e-signature and completion steps to finalize the evaluation.

SEFWE II captures student feedback on the fieldwork educator’s performance and supervision.
