Forms in Learning Activities help you collect evaluations, feedback, and assessments during student placements.
Use forms to:
- Track student performance
- Collect structured feedback from evaluators
- Standardize evaluation workflows across courses and placements
Form Types
Single-term Forms
A single-term form is used once during a specific placement or term. It is tied to a single course offering and supports one-time evaluation.
Example in Exxat Prism
- Final evaluation for a Fall clinical rotation
- End-of-placement site evaluation
When to use Single-term Forms
- When you only need a final assessment at the end of a placement
- When no progress tracking is required during the rotation
Multi-term Forms
A multi-term form is used multiple times within the same placement (for example, midterm and final).
Multi-term forms allows you to track progress and compare performance over time.
You can copy and pre-populate responses across related evaluation forms with the same layouts, reducing repetitive data entry and ensuring consistency across submissions.
To enable copy and pre-populate responses for the form contact your account manager.

- Student competency evaluation done at multiple checkpoints
When to use Multi-term Forms?
- When you want to monitor student progress across the placement
- When both midterm and final evaluations are required
Multi-response forms
A multi-response form allows multiple submissions for the same student or placement. It supports input from more than one evaluator or repeated responses.
- You can Configure Dynamic Columns from the Form Fields.
Administrators can configure dynamic columns in multi-response forms using selected form fields, making it easier to view and analyze response data in a structured, customizable grid format.
- Use Add Form button to add multiple responses.
- You can delete any response within the form as required.
- Students can delete the form when the form is In Progress and Is Pending Review from the immediate next reviewer in the workflow.
- Administrators can delete the form, from the review screen. Administrators can delete the form in any status.
- Students can delete the form when the form is In Progress and Is Pending Review from the immediate next reviewer in the workflow.

When to use
- When multiple reviewers need to evaluate the same student

Standard Forms
A standard form is a prebuilt, system-provided form in Exxat Prism. It is ready to use and cannot be edited or customized.

When to use
- When you want to quickly activate a validated, prebuilt form
- When customization is not required

- You cannot unpublish a form after publishing.
