Overview
During your academic program, you may be required to complete forms, assignments, or evaluations as part of your coursework or clinical placement activities in Exxat Prism.
Forms and evaluations are structured activities created by your program administrators to collect important information related to your learning progress, placement experience, and course requirements. These activities help programs monitor student participation, gather feedback, assess performance, and ensure that program requirements are completed successfully.
Depending on the course, forms and evaluations may be used to:
Document clinical or fieldwork experiences
Submit course assignments or reflections
Provide feedback about placements or supervisors
Complete self-assessments or program evaluations
Track learning activities and milestones
Forms may include different types of responses such as ratings, comments, file uploads, or evaluator selections. Once completed, the responses are submitted and reviewed by the program administrators or evaluators.
Students may be required to complete forms for
Placement Courses – Courses that involve clinical placements, internships, or fieldwork.
andNon-Placement Courses – Classroom or didactic courses that do not involve a placement.
Understanding how to access and complete these forms ensures that you can meet program requirements and submit all required information on time.

Completing Forms and Evaluations
Forms and evaluations are categorized into:
Placement Forms – Forms and evaluations associated with courses that include clinical placements, internships, or fieldwork.
Non-Placement Forms – Forms and evaluations assigned in courses that do not involve a placement and are typically related to classroom or didactic coursework.
While the process of completing these forms is similar, they are accessed through different sections within the Coursework section of Exxat Prism.
You can complete the forms and evaluations from the Coursework section or the Learning Activities section in Exxat Prism.

Completing Placement Forms via Coursework
Log in to Exxat Prism using your university credentials.
From the Exxat Prism Student Dashboard go to My Coursework and click View All Coursework
or
From the Hamburger menu, navigate to Coursework section.In the Coursework page, click the Require Placement tab.
The system displays all placement-related courses you are registered for.
In the Course Details column, click on the Course Offering name for which you need to complete a form or evaluation.
The system opens the Course Details page with the Course Activities tab selected. The Forms/Evaluations section displays all forms and evaluations assigned for the placement along with their Due Date and Status.Scroll down to the Forms / Evaluations section and Click the Form or Evaluation Name to open it.
Complete the Form.
Depending on how your program administrators configured the form, you may see different response types, such as:Rating scales
Comment fields
File uploads
Evaluator selection
On completing the form:
Click Save to save your progress and continue later.
Click Submit when you have completed the form.
Click Clear All to reset responses
Click Download to save a copy of your responses
Additional Options
Complete a Form Multiple Times
If a form must be completed more than once:
Click the Add Form icon.
complete the form and submit the forma as instructed by your program.
Skip a Mid-Term Form
If your program allows it, you may see an option to Skip Mid-Semester.
This allows you to proceed directly to the Final-Term form.
Select an Evaluator
Some forms require you to select an evaluator. Choose the appropriate individual, such as:
Clinical Instructor
Preceptor
Supervisor
Completing Assignments
If your program assigns an assignment:
Click the Assignment Name you want to work with.
Click Upload Files.
Upload the required document.
You can:
Click Save to continue later.
Click Submit once the assignment is complete.
Downloading Submitted Forms
You can download a copy of your completed forms or assignments.
Open the submitted form or assignment.
Click Download.
Note: If you need to make changes after submitting a form: Contact your School Administrator.
Your administrator will un-submit the form, allowing you to make the required changes and submit it again.
Completing Non-Placement Forms via Coursework
Some courses do not involve clinical placements but may still require you to complete forms, assignments, or evaluations as part of your coursework.
In Exxat Prism, these activities are accessed through the Do Not Require Placement section within Coursework.
Follow the steps below to access and complete these requirements.
Log in to Exxat Prism using your university credentials.
From the Exxat Prism Student Dashboard go to My Coursework and click View All Coursework
or
From the Hamburger menu, navigate to Coursework section.In the Coursework page, click the Do Not Require Placement tab.

You can also access forms directly from the Learning Activities section your dashboard.
Click View Learning Activities from the dashboard or go to Learning Activities form the hamburger menu.
The Learning Activities page opens in Forms and Evaluation tab with an accordion view showing:
Didactic courses
Placement courses
Expand the placement or Non-Placement course to view assigned forms and evaluations.
Click the Form Status to open the form.
Complete the Form.
Depending on how your program administrators configured the form, you may see different response types, such as:Rating scales

File uploads
Evaluator selection
On completing the form:
Click Save to save your progress and continue later.
Click Submit when you have completed the form.
