Setup
To integrate a form created on Formstack with Exxat Prism, follow these steps for Setup:

- Log in to the Exxat Prism of your Program.

Obtaining the Access Token:
- To enable integration, you'll need an access token from Formstack.
- Follow the provided instructions to acquire the access token from your Formstack account. Refer to the help document here
- Add the Access token found from Formstack:

Mapping Required Fields:
- Once the access token is obtained, proceed to select the specific form you wish to integrate.
- Select the form
- Map the required fields to ensure that pertinent information such as student details, placements, courses, and rotations are accurately captured.

- As a final step, utilize the provided functionality to preview the form. This enables easy access and ensures that the integration is functioning as intended.

View Summary
- Schools can review data filled in by their students on the evaluation summary page as well as see their responses.
- Select the rotation and courses during setup to filter forms. Only forms relevant to the selected rotation and courses will be displayed.
Select Learning Activities >> Course Offerings >> Select Course >> Form/Evaluations - Forms will start showing at the end with “Formstack –” as Prefix followed by the form name.
- You will see the status of each form as "Available" or "Not Available" based on the student's current placement and other criteria.

- If there are multiple forms filled for the same placement, school administrators can view all filled forms.
View Form
Select Learning Activities >> Course Offerings >> Select Course >> Form/Evaluations >> select Forms/Evaluations with Prefix Formstack >> Select student >> Click Available
- If a form is marked as "Available", you will be able to click and view that form. You will be able to see the question and answer not the options (If any).
