A SEFWE form is a structured documentation and clinical reasoning framework used primarily by Occupational Therapy (OT) students and educators.
It helps students organize client information and apply clinical reasoning in a systematic and evidence-based way.
SEFWE stands for:
S – Subjective
E – Environment
F – Functioning
W – What is the problem (Clinical Impression)
E – Expected Outcomes (Goals & Intervention Plan)
SEFWE forms are commonly used in OT programs, especially during Level II Fieldwork Education, to help students gather, analyze, and synthesize clinical information effectively.
Types of SEFWE Forms in Exxat Prism
There are two SEFWE forms available in Exxat Prism:
- SEFWE I: Used to evaluate the Fieldwork Site
- SEFWE II: Used to evaluate the Fieldwork Educator
SEFWE I - Site Evaluation
Purpose
SEFWE I allows students to evaluate the assigned fieldwork site during Level II Fieldwork.
Workflow
Student evaluates the assigned site.
Completes the evaluation
Signs the form
Selects their Educator/Site Supervisor
Submits the form for review
The Fieldwork Educator:
Reviews the form
Signs it
Submits it
The Faculty / Program Administrator:
Performs final review
Publishes the form for future student access

Future student batches can view aggregated feedback to understand how previous cohorts evaluated the site.
SEFWE II – Educator Evaluation
Purpose
SEFWE II allows students to rate and provide feedback on their Fieldwork Educator.
Workflow
Student evaluates the educator they worked with.
Completes the form
Signs and submits it
The Faculty / Program Administrator:
Reviews the submission
Publishes the evaluation
Visibility After Publishing
Once published, the evaluation is visible in: Program Administrator → Location Evaluations
Importance of SEFWE in OT Programs
1. Alignment with Accreditation Requirements
Accreditation bodies such as ACOTE (Accreditation Council for Occupational Therapy Education) require OT programs to ensure students demonstrate competency in:

Activating SEFWE Forms
To activate SEFWE I (AOTA Student Evaluation of the Fieldwork Experience) and SEFWE II (AOTA Student Evaluation of Fieldwork Experience – Educator), follow the steps below:
Login to Exxat Prism using your university credentials.
From the Dashboard, navigate to the Learning Activity section

Select Setup for the course in which you want to activate the form.
Click the Add (+) icon in top right of the Forms and Evaluations table
.
Select the required SEFWE form.
Configure the form
Turn On the Active toggle.
Choose if this form requires a placement - set to Yes by default.
Set the Publish Date and Due Date:
Using anchor values,
or
By selecting specific calendar dates.
Select the faculty role who should be able to view the form and make changes.
Select the faculty role who should be able to only view the form.
Optional: Click on the Preview link to view the sample evaluation form.
Click Save in the top of the form to activate the form.
The form is now active and available in the Forms/Evaluations table. Click the edit icon in the ACTIONS column to make changes to the form.
Review and Publish SEFWE Form
Login to Exxat Prism using your university credentials.
From the Dashboard, navigate to the Learning Activity section
.
Select Forms / Evaluations for the course in which you want to review the submitted forms.
Select the Form / Evaluation to be reviewed.
Click to expand the form / evaluation.
In the table on the right, click on the Pending School Review status to review the form.
Review the form.
In the right of the form ,
Select an option.
Review - to complete the review.
Un-Submit to Student - To send back to students to make the required changes.
Un-submit to Educator - To send back to Educator to make the required changes
Review and Publish - To complete the review and publish the form.
Enter the Comments if any.
Click Update to complete the review.
Click Audit Log - to see the history of changes made to the form.
Upon approval, the Status of the form Changes to Reviewed and Published (or according to the options selected).
Once published:
The evaluation is linked to the appropriate site and location.
Future cohorts can view aggregated feedback.
