Clinical Internship Evaluation Tool - Version 2.0 (CIET v2.0)

The CIET v2.0 form includes the following sections

• Student and rotation information
• Criteria for evaluation - Each criteria has a set of evaluation questions that includes a rating scale.
• Comments section for each evaluation criteria.

Students use the CIET form to self-evaluate during the rotation.

Clinical Instructors use the CIET form to evaluate student performance, provide feedback, and ensure that the students meet professional standards during the rotation.

Program Administrators use the submitted CIET form submission to review the student performance

It is expected that the student's performance improves over the course of the placement. If the student's performance is not improving, the program administrator can choose to provide academic interventions to the student.

Key features 

• Multi-term evaluation
  The student and the clinical instructors can complete the form evaluation at multiple terms as defined in the settings. 
  Meaning - student and clinical instructors can complete the form evaluation at the mid of the rotation (also known as - Midterm) and at the end of the rotation (Also known as - Final term).  The form can also be setup to evaluate only at the end of the rotation. 
  
  
• School Specific Benchmarks
  CIET v2.0 supports school specific benchmarks or standards to be added for evaluation.  If the school chooses to customize the benchmark to align with school standard, the customization can be done via CIET v2.0. 
  
  
• Training Quiz
  The CIET v2.0 requires students and the clinical instructors to take a pre-requisite quiz (integrated in Exxat Prism) to ensure that the student and the clinical instructor have the adequate knowledge to fill the form. 
  This quiz is to be taken at the first instance when the student or clinical instructor fills the CIET form.  Students and clinical instructors can retake the quiz as many times as necessary until they pass. 
  
  
• Parallel Form Completion
  The students and clinical instructors can fill the form parallelly. That is, the student and the clinical instructors can use the form link to complete the form at the same time. 
  However, the students' ratings are not visible to the clinical instructor and vice versa while they are filling the form. This ensure there is no bias while evaluating the form.
  Once both the student and clinical instructor complete the midterm / final evaluation, both the student and the clinical instructor can view the other person’s evaluation
  
  

CIET v2.0 Concurrent Workflow

1. Program administrator makes the CIET v2.0 form available for mid-term evaluation.
2. Student selects the clinical instructor who needs to evaluate the student's performance for mid-term. 
3. Clinical instructor receives an email with the link to the form. 
4. Clinical instructor evaluates the form and submits for program review. 
5. Student self-evaluates the form and submits for program review.
   Note:  Steps 3 and 4 can take place at the same time or at different time.
   Clinical Instructor and students can fill the form simultaneously.  Student or the Clinical Instructor cannot see the evaluation results of each other until both of them submit the evaluation form. 
6. Program administrator reviews the form submitted.
7. Program administrator enables the form for final term evaluation. 
8. Student selects the clinical instructor who needs to evaluate the student's performance for mid-term. 
9. Clinical instructor receives an email with the link to the form. 
10. Clinical instructor evaluates the form and submits for program review. 
11. Student self-evaluates the form and submits for program review.
    Note:  Steps 10 and 11 can take place at the same time or at different time.
    Clinical Instructor can fill the form simultaneously.  Student or the Clinical Instructor cannot see the evaluation results of each other until both of them submit the evaluation form. 
12. Program administrator reviews the submitted form. 

Activating CIET v2.0 Form

The program administrator should activate the form and distribute it to the students placed in the desired course. 
To activate the CIET v2.0 form for evaluation follow these steps. 

1. Login to the Exxat Prism and go to the Courses section in the dashboard. 
   
   You can also go to the courses section from the hamburger menu.
   
2. Select the desired course from the Course Offerings list.
   
3. In the left-hand menu, go to Learning Activities section, and go to Setup. 
   
4. In the Forms / Evaluations grid, click on the Add icon. 
   
   Forms/Evaluations pane opens to allow you to configure the required form. 
5. Select CIET v2.0 form from the list. 
   You can also search by the form name in the search bar.
   
   
   
6. Configure the CIET v2.0 form
   1. Does this form require a placement? - By default, the answer is "yes". You can choose "No" if it is not applicable for CIET v2.0 form.
   2. Choose the CIET v2.0 form WorkFlow drop-down menu.
      • CIET v2.0 Midterm and Final: Student and Clinical instructor evaluate the student’s performance once in the midterm of the student’s placement and once at the end of the student’s placement. 
      • CIET v2.0 - Final: The final version of the form only allows the student and clinical instructor to evaluate the student's performance once. That is at the end of the placement.
        
        
   3. Configure the Publish and Due date for the form
      The publish dates and end dates will depend on the dates of the student's placement. Program Administrators will indicate the number of days, before or after a specific placement related anchor point the program wants the CIET version 2 form to be active and the date for which it is due.
      
      For example:
      If the program would like to publish the form to students, clinical instructor, and the program 10 days after the placement mid date.
      1. Enter 10 in the Days box in the Publish date for form is setting.
      2. Click on the Before or After dropdown and choose the After option.
      3. Click on the Anchor dropdown and select the Placement Mid Date option.
         Configure the Due date for the mid form is and Due date for the Final form is settings in the same way.
   4. Select the faulty role who can view and edit the form. 
   5. Select the faculty role who can only view the form. 
      
   6. Select if you want to refer to another activated form while filling this form. 
7. When the configuration is complete the Active Status of the CIET v2.0 is active.  Ensure this is Active before you save the form configuration. 
   
8. In the top right of the Form / Evaluations pane, click the Save button.

The form is activated and is available in the Forms/Evaluations grid.

Completing the CIET v2.0 Form 

Student's self evaluation

The student needs to follow these steps to complete the self evaluation in the CIET v2.0 form.

1. Login to Exxat Prism, and Click View Learning Activities to go to the learning activities section.
   
   Students can also go to the Learning Activities section from the hamburger menu.
   
2. In the Forms / Evaluation tab click on the placement accordion to view the forms/evaluations published for the placement. 
   Student will have an accordion for each of their placements.
   
   Tip: Students can also use the search box in the top-left of the page to search for CIET 2.0 form. 
3. Click on the CIET v2.0 form status Get Started to start self-evaluation. 
   
   
   • If the student is filling the form for first time, then the student has to pass the CIET 2.0 form training quiz. 
     
   • Students can take the quiz any number of times until they pass the quiz.
   • Once the student clears the training, the student need not take the quiz again, even though the student is filling the quiz second time.
   • School defined customized benchmarks added by the program will appear in the instructions section. Click view instructions link in the banner on the top, to see instructions.
     
4. Select the Clinical Instructor who supervised Clinical Placements and click Send button. 
   The Clinical Instructor will receive an email with a link to access the CIET v2.0 form and evaluate the student. 
5. Click on the CIET v2.0 form status In Progress to complete the self-evaluation.
   
6. Students evaluate themselves for Physical Therapy behaviour attributes using a rating scale foreach evaluation. Students can also add comments for each section. 
   
   
   
   • Use Clear All button in the top to clear evaluations and comments made in the form. 
   • Use Save button to save the progress of the self-evaluation. 
   • Use Submit button to submit the form.
   • Use the Resend button to resend the form access link email to the Clinical Instructor. 
7. Provide Global Rating of Student Clinical Competence rating at the bottom of the form. 
   
   Ensure to fill the form completely before submitting the form. Once you submit the form the form status changes to Submitted by Student.

Clinical Instructors evaluation of the student

1. Clinical Instructor uses the link emailed to them to access the CIET v2.0 form to evaluate the student. 
   
2. Clinical Instructor should select Yes, if the instructor has taken the prerequisite training quiz earlier, and select No, to take the quiz. 
   
3. Clinical Instructor evaluates the student by selecting the rating scale for evaluations, provide comments for each section and clicks Submit. 

Note: In CIET v2.0 both Student and the Clinical Instructor can fill the form simultaneously.  Student or the Clinical Instructor cannot see the evaluation results of each other until both of them submit the evaluation form. 

Administrator's review of the CIET v2.0 form 

Once both student and clinical instructor submit the form, The status of the form changes to Midterm-Pending School Review. 

If the form was setup to be reviewed by faculty members, then the faculty can review the form. 

1. Click on the status column to begin review of the evaluations submitted by student and clinical instructor. 
2. Administrator can override the student or clinical instructors evaluation if required. 
3. Administrator can also un-submit the form to student or clinical instructor for the respective stake holders to go back and resubmit the form. 
   Click Select an Option drop-down in the right of the page. 
   
4. Click Review from the Select an Option drop-down to complete the review. 
   
5. Click Update to apply the review changes. 

If the form was only set up for one term, the workflow of the form would end here and nothing else would need to be completed. 

If the midterm and final workflow was selected by the program, the form status will then change to final get started.
Student and Clinical Instructor will follow the same process as in Midterm evaluation.

Reports to run on the CIET v2.0 form 

Raw Data Report and Graphical Report for Placement forms consolidates all the response to form in the report. 

Raw Data Report

Raw data report can be used to view all the responses to the CIET Version 2 for specific courses, rotations and settings.

1. Go to Learning Activities form the hamburger menu. 
2. Go to Reports.
   
3. Click on the Forms / Evaluations  Raw data Report
   
   
4. Select the Course Offerings and select the Filters as applicable. 
   For example: Select CIET v2.0 for Forms/Evaluations filter.
   
   
5. Click Generate Report

Graphical Report

1. Go to Learning Activities form the hamburger menu. 
2. Go to Reports, select Graphical Report for Placement Forms.
   
3. Select the Course Offerings and select the Filters as applicable. 
   For example: Select CIET v2.0 for Forms/Evaluations filter.
