In Exxat Prism the CI (Clinical Instructor) Details form is used to collect and manage information about the Clinical Instructors associated with the site or rotation.

This form ensures all the information about the instructor is documented, verified, and is accessible.

CI details form is used to assign instructor while scheduling the rotation. CI details form is also used to manage licensure renewals and check instructor eligibility.

Activate Form

To activate the CI details form for a course, follow these steps.

Login to the Exxat Prism and go to the Learning Activities section in the dashboard or use the hamburger menu.
From the Course Offering tab click on the course for which you want to activate the CI Details form.
From the left menu, go to Learning Activities and click Setup.
In the Forms / Evaluations grid, click the "+" icon in the top right for the grid.
In the Forms / Evaluations drawer go to CI Details.
Configure the form
- Enable the Active slider.
- Choose if the form requires a placement.
- Set a date for when the form needs to be published.
- Select the faculty roles.
Click Save.

The activated form appears in the Forms / Evaluations grid for the course.

Click the pencil icon in the corresponding ACTIONS column to make changes to the form settings.

Review Form

From the Course Offering tab click on the course for which you want to review the CI Details form.
From the left menu, go to Learning Activities and click Review
Select CI Details in the Forms / Evaluation Summary in the left menu.

Click on the Pending School Review STATUS for the corresponding student.
Review the form, once review is complete select the Reviewed option from the Select an Option section in the right and click Update.

You can un-Submit the form to the student if require. Select Un-submit to student and click Update.
