Experiential Supervisor Intake Form

Introducing the Experiential Supervisor Intake Form in Exxat Prism. The new Experiential Supervisor Intake Form workflow enables program administrators to send intake forms directly to Experiential Supervisor to collect information,  review submitted information, and sync updates directly to preceptor profiles from a centralized workflow.

Program administrators can use a comprehensive standardized form to capture key  Experiential Supervisor details and add custom sections for program-specific information. The workflow also supports response tracking, reminders, document uploads, and review before syncing updates to profiles.

Experiential Supervisors can complete forms without creating an account by using a delegator link. Initial requests are pre-filled using existing school data, and subsequent requests can reuse previously submitted information, reducing the need to repeatedly complete the form from scratch.

This enhancement simplifies the review and sync experience by reducing manual data entry, improving data accuracy and consistency, streamlining preceptor onboarding and profile management, and helping maintain up-to-date preceptor records more efficiently.
