Improve Your Placement Process with the New Experiential Supervisor Intake Form
This webinar introduced the new Experiential Supervisor Intake Form in Exxat Prism, a configurable tool that allows program administrators to collect comprehensive supervisor information directly from clinical instructors, fieldwork educators, and other experiential supervisors. Presented by David (Project Manager, former DCE) with Pratiksha (Product Manager) supporting in the chat, the session walked through form setup, the preceptor completion experience, and the admin review/sync workflow.
Key Takeaways
-
Configurable but standardized structure — Admins can mark all standard section fields (for example: fields within basic details, professional summary, licensure, supervision capacity, discipline-specific cards) as mandatory or optional, and add custom questions. Standard sections cannot be hidden, by design, so preceptors fill out one comprehensive form rather than different versions per program.
-
"Fill once, use everywhere" — When preceptors check this box, their responses save across Exxat Prism programs, so the next school that sends them the form gets pre-populated answers (except custom questions, which are program-specific).
-
Review and sync workflow — Responses don't auto-overwrite the personnel profile. Admins review responses side-by-side (preceptor answer on left, profile field on right), can edit before syncing, and changes vs. previous values are flagged. Practice site associations still require a manual step because site naming varies by program.
-
Custom questions are permanent once added — They can be hidden via the visibility toggle but cannot be deleted or edited after being sent, to protect data consistency across submissions. Plan custom questions carefully.
- Roadmap priorities (based on poll results) — Bulk sending (50%) was the top requested enhancement, followed by sending from the slot request/clearance dashboard (27%), profile update notifications to schools (16%), and student-initiated sending (9%). The product team is continuing to update and enhance this new feature.
